Northwestern University Feinberg School of Medicine (Feinberg), Northwestern Memorial HealthCare (NMHC), Children’s Hospital of Chicago Medical Center (Lurie Children's), and the Shirley Ryan AbilityLab (SRALAB) are committed to conducting their business with integrity and transparency. A cornerstone of this commitment is the management of conflicts of interest through appropriate disclosure and the development of effective conflict management plans.

Per our conflict of interest policies, you are required to participate in this process by completing an online annual disclosure survey as well as updating your responses when changes occur. The survey combines questions from Feinberg, NMHC, Lurie Children's, and the SRALAB, resulting in the completion of one survey. You will receive a customized survey that reflects all the roles you have.

The instructions on how to log into the survey site will be distributed to via email on February 1, 2024. All users will be required to complete the annual disclosure survey on or before February 29, 2024.

Your responses to the survey will be shared with the appropriate individuals within and across organizations as necessary for the purpose of reviewing and resolving conflicts of interest. Your responses may be verified by contacting third-party sources (e.g., research sponsors or companies with which you may have a consulting relationship). If you disclose an arrangement or relationship that may fit the definition of a conflict of interest, you may be contacted to discuss the relationship and, if necessary, to determine an appropriate course of action.
