Acceptance Checklist
Before arriving, you will need to submit the following required forms by the listed deadlines. You are also expected to review the resources below and the information found on the First Day page.
Domestic Students
Rotation Fee Payment: Due within 14 days from the date of your acceptance offer. Confirmation Form: Due after rotation fee has been paid.
Global Partner Students
- Rotation Confirmation Form: Due within 14 days from the date of your acceptance offer
- Arrival and Travel Form: Four weeks prior to the start date of your rotation
- Insurance Payment: Two weeks prior to the start date of your rotation, you will receive the exact cost of your insurance and a link to the online payment portal.
All Visiting Students
Cancellation or Withdrawal Form: Complete this form to cancel an accepted elective or drop an elective that you have started. Submitting this form and dropping the rotation in VSAS (if applicable) is required to cancel an elective. It must be received no later than 4 weeks prior to your start date. Failure to submit this form on time will result in being dropped from this and future rotations at Northwestern, and your school will be notified.
